Recovery Care at Home – Common Points of Entry
Direct from the Emergency Department (ED)
Our Recovery Care Coordinators work with emergency room teams to identify patients who meet clinical criteria for in-home recovery. With physician approval, eligible patients are given the choice to continue their care at home—an option that over 90% of our patients gladly accept.
After a Hospital Admission
Patients already admitted to a hospital’s general medical floor may be discharged earlier to continue their treatment at home. Once their condition stabilizes, they are transitioned into our Recovery Care at Home program to finish their care plan in a more personal and supportive environment.

- Patients admitted to the hospital can also be referred to our program once they are stable enough to transition home.
